1. Promotion Overview

By participating in this promotion, you agree to the following terms:

• Purchase any Elevit Preconception & Pregnancy or Menevit at any participating Priceline Pharmacy store within Australia or online at https://www.priceline.com.au/ between 12:00 am AEST on 10 September 2026 and 11:59 pm AEST on 31 December 2026.
• Enter the Batch / Lot number from your eligible Elevit or Menevit product packaging when prompted on the Lumi Health website (https://www.lumihealth.com.au/elevit-priceline).
• Order any Lumi Health Carrier Screening test options, including the Medicare bulk-billed Standard Carrier Screening test.
• Complete your Lumi Health onboarding steps and receive a Prenatal Wellness report outlining insights to support you in preparing for pregnancy and recovery. The Lumi Health Prenatal Wellness report is valued at RRP $59.95.
• Tests must be received by 11:59 pm AEST on 31 January 2027 to receive the Prenatal Wellness report.

2. Eligibility

• Open to Female (assigned at birth) Australian residents aged 18 and over.
• Individuals who wish to claim Medicare bulk billing for the Lumi Standard Carrier Screening Test must hold a valid Medicare card, be female assigned at birth, and be pregnant or planning a pregnancy.

3. How to Redeem

• Purchase an eligible Elevit Pre-conception & pregnancy or Menevit pack at any participating Priceline Pharmacy Store within Australia or online at priceline.com.au between 12:00 am AEST on 10 September 2026 and 11:59 pm AEST on 31 December 2026.
• Enter the Batch / Lot number from your eligible Elevit or Menevit product packaging when prompted on the Lumi Health website (https://www.lumihealth.com.au/elevit-priceline).
• Order any Lumi Health Carrier Screening test via Lumi Health.
• Complete your Lumi Health onboarding steps and receive a Prenatal Wellness report outlining insights into Iron absorption and transport, Vitamin D production and conversion, predisposition to weight gain during pregnancy and predisposition to elevated blood glucose levels. The Lumi Health Prenatal Wellness report is valued at RRP $59.95.
• Return your Prenatal Wellness sample together with your Carrier Screening sample using the Reply Paid envelope provided in your Carrier Screening Kit. Both samples must be received by 11:59 pm AEST on 31 January 2027 to receive the Prenatal Wellness report.

4. Promotion Period

• A purchase of Elevit Pre-conception & Pregnancy or Menevit vitamins at any participating Priceline Pharmacy Store within Australia or online at www.priceline.com.au, between 12:00 am AEST on 10 September 2026 and 11:59 pm AEST on 31 December 2026.
• Lumi Health carrier screening onboarding and swab sample must be received by 11:59 pm AEST on 31 January 2027 to receive the Prenatal Wellness report up to 2 weeks after the sample is received at our lab.

5. Prenatal Wellness Report Details

Exclusive offer to customers from Priceline Pharmacy and Elevit.

The Lumi Health Prenatal Wellness report assesses key genes that may indicate focus areas for women planning or expecting a baby.

• The insights in the report will outline whether your genes indicate the area of focus when preparing for pregnancy. The report provides recommendations; it is advisable to speak with your GP and relevant healthcare practitioners before making any changes to your diet or supplementation.
• The Prenatal Wellness report is not transferable, exchangeable, or redeemable for cash or any other products or services.

6. General Conditions

• The participants agree and acknowledge that they have read these Terms and Conditions, and that entry into the promotion is deemed to be acceptance of these Terms and Conditions.
• The Promoter reserves the right to verify the eligibility of participants (e.g., Medicare/residential status and completion of the telehealth call). In the event that a participant cannot provide suitable proof as required by the Promoter to validate their claim, the participant will forfeit the offer in whole and no substitute will be offered.
• If there is a dispute as to the identity of a participant, the Promoter reserves the right, in its sole discretion, to determine the identity of the participant.
• The Promoter reserves the right to disqualify any claims or individuals who do not comply with these terms and conditions or who engage in fraudulent or manipulative activity. If there is a dispute regarding the conduct of the promotion or the claim of an offer, the Promoter will resolve it in direct consultation with the participant. If the dispute cannot be resolved, the Promoter’s decision will be final.
• The Promoter reserves the right to modify, suspend, or cancel the promotion or these terms and conditions at any time due to unforeseen circumstances or as deemed necessary. Reasonable notice will be provided where possible.
• The Promoter accepts no responsibility for any tax implications that may arise from participating in the promotion. Participants should seek their own independent financial advice.
• Any guarantee or warranty given is in addition to any relevant statutory guarantees and warranties, and nothing in these Conditions of Claim restricts, excludes or modifies or purports to restrict, exclude or modify any statutory consumer rights under any applicable law including the Competition and Consumer Act 2010 (Cth).
• If for any reason any aspect of this Promotion is not capable of running as planned, including by reason of computer virus, communications network failure, bugs, tampering, unauthorised intervention, fraud, technical failure or any cause beyond the control of the Promoter, the Promoter may in its sole discretion cancel, terminate, modify or suspend the Promotion and invalidate any affected claims, or suspend or modify the provision of discount codes, subject to State or Territory regulation.
• Participation in this promotion does not alter, waive, or supersede the standard Lumi Health Terms & Conditions governing testing services, lab processing, customer eligibility, or data privacy. All Lumi Health tests redeemed through this promotion are strictly subject to Lumi Health’s standard terms of service.

7. Liability & Privacy

Lumi Health and Bayer Australia are not responsible for:

• Lost or stolen testing Kits.
• The availability of Elevit Preconception & Pregnancy or Menevit Multivitamins at Priceline Pharmacy stores.
• Technical issues that may affect a participant's ability to order a Lumi Health Carrier Screening Test, complete the telehealth call, or receive the Prenatal Wellness report.

Personal information collected by Lumi Health as part of this promotion will be managed in accordance with the Lumi Health Privacy Policy, available at Lumi Health Privacy Policy. By participating in this promotion, you consent to the collection and use of your personal information in accordance with that policy for the purposes of administering the promotion. The claimant agrees to the Promoter sending them electronic communications regarding this Promotion (for administrative purposes only) and acknowledges that such communications may not have an unsubscribe facility. If the claimant does not provide their personal information as requested, they may be ineligible to submit a claim for a gift in the Promotion.

The Promoter and its associated agencies and companies will not be liable for any loss (including, without limitation, indirect, special or consequential loss or loss of profits), expense, damage, personal injury or death which is suffered or sustained (whether or not arising from any person's negligence or wilful misconduct) in connection with this promotion or accepting or using any offer (or recommendation).
